Abstract: 

در ایالات متحده آمریکا، تروما شایع ترین علت مرگ و میر زیر 40 سال می باشد و حدود نیمی از مرگ و میر آن، مربوط به ضربه سر است. در بیمار با آسیب دیدگی حاد، توجه به راه هوایی، علایم حیاتی، قفسه سینه، خونریزی و بی حرکتی ستون فقرات، قبل از مغز اهمیت دارد.بازآموزی مواجهه با ضربه متعدد و ضربه سر برای کلیه کارآموزان، کارورزان، پزشکان عمومی، دستیاران و متخصصین رشته های جراحی توصیه می شود.اهداف مقاله:1- افزایش آگاهی به اهمیت راه هوایی، تنفس و گردش خون در تروما.2- مخاطب بتواند نحوه برخورد به آسیب راه هوایی، تنفس و گردش خون در تروما را بیان کند.3- افزایش آگاهی به جایگاه جراحی عمومی و جراحی مغز و اعصاب در تروما.4- مخاطب بتواند نحوه برخورد به آسیب دیدگی های مغز را بیان کند.با مطالعه دقیق این مقاله، مخاطب باید بتواند به حداقل 80 درصد سوالات مطرح شده پاسخ درست بدهد.

Abstract: 

The volume and diversity of data in the world are unprecedented in human history. It is growing at an unprecedented rate. Internet and social media technologies as they permeate every stage of our lives and even our mobile phones, people have become a source of data even in their daily activities. So, a new concept emerged: "Big Data". Big data is produced with high volume, speed, structured diversity, and semi-structured and unstructured data. Many industrial areas release big data by creating new data or digitizing existing data models so that organizations can gain a competitive advantage. In order to extract economic value from big data, it should be processed with advanced analytical methods. This research aims to examine the use of Hadoop in analyzing big data according to the Map-Reduce model. and distributed file systems such as Processing, PIG, Mahout, NoSQL, and Cassandra, and the study concluded that advanced analytical methods protect the privacy of personal information, and through them, security gaps can also be filled, and the phenomenon of big data was discussed in terms of its components and resources, and it was emphasized on the advantages of big data in the areas of application.

Abstract: 

Purpose: To report a patient with MEWDS.Patient and findings: A 65-year-old man presented with sudden decrease of vision in his right eye. On ophthalmoscopy of the involved eye, numerous small discrete white lesions were detected superficially and deep in the retina around the disc and macula. On visual field examination, enlargement of the blind spot was detected. Fluorescein angiography was unremarkable. On electroretinography, the a-wave was decreased minimally. The patient was followed and after 2 months, significant improvement of visual acuity and visual field was observed.Conclusion: MEWDS is a rare intraocular inflammatory disease of unknown etiology and in contrast to our patient, more frequent in females. The natural course is generally benign with spontaneous disappearance of the lesions and improvement of vision in 6 to 8 weeks.

Abstract: 

Impleader claim is one of ancillary suits which permit to the parties to implead third person to the action and became that person one of parties. The Iran civil procedure code, unlike some other countries for example Lebanon no attention to purposes of Impleader claim and in Article 135 the subject is expressed in general terms. With attention to generally speaking of this Article and referring to some other rules such as Considering third person as a party of the claim, preventrenewed and repeated claims and as a result, to eliminate the issuance of conflicting orders, It can be said that in a wide range of casesThere is possibility of usingImpleader claim. Therefore, the original plaintiff may implead third one and seek his conviction to original or related trials demand, also the defendant with impleader claim, may cause third persons convictionIn favor ofhimself(defendant) or plaintiff. In addition, impleader claim can be a preliminary and auxiliarymeans to win the principal lawsuit witch sometimes mentioned as auxiliary impleader claim.

Abstract: 

Purpose: To report a case of severe macular edema following the taking of fingolimod in multiple sclerosis.Case Report: A 34-year-old woman with a long history of relapsing-remitting multiple sclerosis presented complaining of blurred vision in the left eye that started 1 week before the presentation. She had been taking fingolimod for 4 months. She was not diabetic and had no history of uveitis but reported an episode of retrobulbar optic neuritis in both eyes. The initial ophthalmic examination, which was performed 2 weeks before initiating fingolimod, revealed a best corrected visual acuity (BCVA) of 20.25 OU and no ocular abnormalities. Upon the recent presentation, BCVA was 20.25 OD and 20.70 OS and there was no afferentpapillary defect. The examination of anterior segment and anterior vitreous was unremarkable. Retinal examination of the left eye revealed cystoid macular edema with no other inflammatory signs or vascular changes. The findings of spectral domain optical coherence tomography (SD-OCT) were compatible with macular edema and indicated that the central macular thickness (CMT) was 605 microns. Fingolimod was discontinued and sinarolac was started three times daily. After 6 weeks, BCVA was 20.25 OD and 20.30 OS.Macular edema was resolved clinically and SD-OCT exhibited a significant reduction in macular edema with a CMT of 277 microns.Conclusion: Fingolimod-associated macular edema is a benign complication of the drug used for multiple sclerosis and. This complication responds well to the drug discontinuation and topical non-steroidal antiinflammatory administration.

Abstract: 

Introduction: Myocardial bridging is a congenital condition in which a segment of a major epicardial coronary artery has an intra myocardial route. Myocardial bridging is usually confined to a single vessel and is typically the mid segment of the left anterior descending artery. Bridging may be associated with chest pain, myocardial infarction, and sudden cardiac death, or usually asymptomatic.Case Report: A 59 year-old women with diabetes mellitus and hypertension presented with dyspnea and chest pain on exercise. Physical exam, electrocardiography and echocardiography were intact. In angiography, we found severe stenosis of left main coronary artery and severe stenosis of three coronary artery (Three Vessel Disease) as well as multiple myocardial bridges at the end of right coronary artery.Conclusion: Myocardial bridging is a rave condition, however, it be considered in patients facing recurrent chest pain, it has severe complication such as sudden cardiac death.

THERE IS NO DOUBT THAT ENERGY AND FRESH WATER ARE TWO IMPORTANT INGREDIENTS OF SOCIO-ECONOMIC DEVELOPMENT AT ANY REGION. ACCORDING TO VERY LIMITED ACCESS TO FRESH WATER AT MANY PLACES AROUND THE WORLD, TODAY, USING DESALINATION PLANTS TO PRODUCE POTABLE WATER IS A COMMON SOLUTION TO SUPPLY REQUIRED FRESH WATER ALL AROUND. ON THE OTHER HAND, ALL DESALINATION PLANTS NEED ENERGY (HEAT OR ELECTRICITY) TO PRODUCE FRESH WATER, AND WITH REGARD TO CONVENTIONAL ENERGY RESOURCES LIMITATION IT SEEMS THAT THE COMBINATION OF RENEWABLE ENERGIES WITH DESALINATION TECHNOLOGIES WILL PLAY A UNIQUE ROLE IN ENERGY AND WATER MANAGEMENT IN THE NEAR FUTURE. IN THIS PAPER AN ATTEMPT HAS BEEN MADE TO PROPOSE APPROPRIATE SUSTAINABLE ENERGY SOURCES FOR EACH KIND OF DESALINATION SYSTEM (MEMBRANE OR THERMAL). IT IS CLEAR THAT MEMBRANE TECHNOLOGIES, LIKE RO DESALINATION PLANTS, NEED ENERGY IN THE FORM OF ELECTRICITY WHILE THERMAL TECHNOLOGIES, SUCH AS MED AND MSF SYSTEMS, USE HEAT AS THE ENERGY SOURCE. ACCORDINGLY, IT IS IMPORTANT TO RECOGNIZE THE SUITABLE ALTERNATIVE ENERGY SOURCE FOR EACH DESALINATION SYSTEM.

Abstract: 

Saliva, as a complex fluid composed of secretory products from salivary glands, plays different roles in the oral cavity. Saliva has potentially been used for the diagnosis of viral, bacterial, and systemic diseases in the last few years. Individuals with minimal training can collect saliva as a diagnostic fluid easily and non-invasively. Saliva is a simple and economical method for screening the public. However, saliva has various levels of variability and instability. A systematic search of published literature was performed on Medline using PubMed. Several keywords, including saliva, systemic disease, disease, health, and diagnosis, were used in this search. This review discusses the diagnostic potential of salivary systemic diseases such as infectious, malignant, and other systemic diseases.
